The concern may be reported to the Laboratory Investigations Unit_[2].

As indicated in the brochure, please include the following information in your correspondence:

• Name/address of laboratory
• Who was involved/affected
• Description of the concern
• Date/time of incident
• Frequency of the incident
• Other agencies contacted
• Your contact information (unless you prefer to remain anonymous)
