Report a concern about a laboratory

Please include the following:

  • Description of the concern
  • Name/address of the laboratory
  • Date incident occurred
  • Name of individuals involved/affected
  • Other agencies to whom the incident was reported
  • Estimate of the frequency or pervasiveness of the issue
  • Your contact information (if you wish to not remain anonymous)

Your concern may be provided to ELAP by phone, email, fax or mail as indicated to the right or to the Laboratory Investigations Unit by phone at 1-800-682-6056.
