Please include the following information when reporting a concern about regulated medical waste:
- Description of the concern
- Name/address of the facility
- Date incident occurred
- Name of individuals involved/affected
- Other agencies to whom the incident was reported
- Estimate of the frequency or pervasiveness of the issue
- Your contact information (if you wish to not remain anonymous)
The concern may be reported to the Laboratory Investigations Unit or by phone at 1-800-682-6056.
