To view your laboratory information and verify your CLIA certificate, please utilize the CLIA search lookup tool QCOR found at https://qcor.cms.gov/main.jsp. On the left, please select CLIA Laboratory Lookup. From there, enter your CLIA number and select search. Your laboratory demographics will be found in the box along with a link to view your most recent electronic CLIA certificate. Changes are reflected in QCOR database without a revised CLIA certificate being generated.
There is no CMS fee for printing out a CLIA certificate from the link in QCOR.
Information at QCOR is updated one day after changes to certificate information is entered in Aspen Web by POLEP.
The CLIA certificate link on QCOR will NOT be updated UNLESS the facility requests a revised CLIA certificate. The laboratory will be assessed a fee for this revised CLIA certificate. Once paid the link will reflect the updated CLIA certificate. The link to the certificate in QCOR will indicate prior demographic data unless a revised certificate is requested/generated.
